I, the undersigned LVBARSTARZZ 5K RUN (hereinafter “EVENT”) participant hereby release and discharge event holder LVBARSTARZZ LLC and all LVBARSTARZZ LLC event volunteers and organizers from any and all liability, claims, demands, or causes of action that I may hereafter have for injuries, damages, or death arising out of my participation in EVENT, including but not limited to losses caused by the negligence of LVBARSTARZZ LLC and its event volunteers and organizers. I understand and acknowledge that EVENT activities have inherent dangers that no amount of care, caution, instruction, or expertise can eliminate and I expressly voluntarily assume all risk while participating in the EVENT activities whether or not caused by the negligence of the released parties.

Parental Waiver and Release Form

As the parent or legal guardian of the child named below, I hereby hereby give my full consent and approval for my child to participate in the EVENT and agree to this Waiver and Liability Release and am willing to assume these risks on behalf of my child. I hereby certify that my child is fully capable of participating in the EVENT and that my child is healthy and has no physical limitations that would restrict full participation in any and all EVENT activities. Accept waiver as parent/guardian on behalf of a minor
